Myasthenia gravis (MG) is an autoimmune disease usually caused by antibodies that block or destroy receptors for the neurotransmitter acetylcholine, leading to muscle weakness and fatigue.
Tests generally appear in the order most useful for common clinical situations
Test name: Acetylcholine Receptor Antibody Reflexive Panel
ARUP #: 2001571
Methodology: Quantitative Radioimmunoassay/Semi-Quantitative Radioreceptor Assay
Use: Assist in diagnosis and monitoring of MG; panel contains binding, blocking and modulation antibodies
Limitations: Approximately 85-90% of patients with MG express antibodies to AChR, which can be divided into binding, blocking, and modulating antibodies Approximately 10-15% of individuals with confirmed MG have no measurable binding, blocking or modulating antibodies Treatment may alter test results
Test name: Myasthenia Gravis Panel (Acetylcholine Receptor Antibody Reflexive Panel and Striated Muscle Antibody, IgG with Reflex to Titer)
ARUP #: 2002666
Methodology: Semi-Quantitative Indirect Fluorescent Antibody/Quantitative Radioimmunoassay/Semi-Quantitative Radioreceptor Assay
Use: Assist in diagnosis of MG, particularly when related to thymoma
Test name: Titin Antibody
ARUP #: 2005636
Methodology: Semi-Quantitative Enzyme-Linked Immunosorbant Assay (ELISA)
Use: May assist in diagnosis of seronegative MG Helpful if thymoma suspected Monitor disease status and evaluate therapy; use in conjunction with acetylcholine receptor in management of patients
Test name: Acetylcholine Receptor Modulating Antibody
ARUP #: 0099521
Methodology: Semi-Quantitative Radioreceptor Assay
